V. Services for the Older Persons - Community Support Services

Objectives:
Community support services assist elderly people to remain living in the
community for as long as possible. These services also give support to carers.
With increasing diversity of community support services for the elderly, a more
integrated approach to facilitate access to services is adopted

Provides
community support service, including home help service, counseling,
continuous education program, social and recreational service, community
education, laundry, bathroom and canteen service for old people in the

Support Team

Provides
social networking and volunteer services to the vulnerable elderly.
The teams also develop volunteer services for the older persons to contribute
to the society by taking part in the volunteer work. |

Provides
neighborhood support services to the elderly by rendering social and
recreational service, counseling, networking and elderly learning |

Day Care

personal care and limited nursing care to frail elderly living in the
community during the daytime. |

Day Respite Service |
Provides temporary day care service for demented or frail elderly people so that their carers can have be relieved from the longstanding care tasks.

12 Day care centres for the elderly are providing day respite service for the elderly and each center offers 3 places.

Dementia Day Care Centre |
Provides day care service and training to elderly people suffering from mild to moderate grade senile dementia as well as to provide supportive and counselling service to carers to release their burden. |
4 centres and each provide 20 places. |
Home Help Service |
Aims at assisting the elderly, the disabled, individuals and families who are incapable of looking after themselves or are unable to maintain the normal functioning of their households. It provides personal care, household management, meal delivery, escort service, child-minding ,etc |

Special Nursing Care:, such as continence care, respiratory care,
diabetes care, infection control -
Personal Care, such as transfer, food feeding, bathing , hair
washing, nail cutting -
Maintenance rehabilitation and restorative exercise -
Support Services e.g. accident prevention, disposal of medical
waste -
Carer support service -
Day and home respite -
24-hour emergency support -
Environment risk assessment and home modification -
